The High-Deductible PPO 3000 or 5000 are high-deductible medical insurance plans with no co-pays for office visits, chiropractic care, hospitalization, home health care, and physical/occupational therapy, after the deductible is met. Both include twenty-percent prescription co-pay and allow you to open a tax-advantaged Health Savings Account.
The PPO 1000-5000 health care plans are four different health care plans that have deductibles ranging from one thousand dollars to five thousand dollars for individuals and two thousand dollars to ten thousand dollars for families. These plans all include eighty percent coverage for hospitalization, physical/occupational therapy, chiropractic care, and home health care with a yearly maximum. Prescription co-pay costs range from fifteen to forty dollars.
The First Dollar PPO 35 health insurance plan offers sixty-five percent coverage for hospitalization, physical/occupational therapy, chiropractic care, and home health care, with a yearly maximum. Prescription drug co-pay costs range from fifteen to forty dollars.
The Preventative and Hospital Care 1250 and 3000 are two insurance plans that offer eighty percent coverage for hospitalization, home health care, and skilled nursing, with a yearly maximum. With each of these plans office visits, durable medical equipment, physical/occupational therapy, and chiropractic work are not covered.
